This professional position provides care management services to older adults. Duties include: assessing social, emotional, economic, health & physical needs, and living conditions of older adults; collaborating with customers, their family members, medical providers, and/or other agencies to develop an appropriate care plan to meet identified needs; and coordinating and securing services as planned.  Ongoing case monitoring and periodic reassessments are performed by employee to ensure customer needs are met. Care manager maintains complete and current information which is obtained during this process.  The essential functions of this position are underlined throughout this job description.
