JOB SUMMARY: The Palliative Care Clinical Coach Reports to and works under the direction of the Home and Community Care Manager along with the Six Nations Wellbeing Department, Home and Community Care
The Palliative Care Clinical Coach is responsible for driving practice change, with a focus on improving access to and quality of clinical care, among participating community organizations (i.e. primary care) in alignment with provincial direction established by Ontario Health and guidance developed by the Ontario Palliative Care Network. This position has a functional relationship with working with Six Nations of the Grand River community palliative care, Ontario Health and the Ontario Palliative Care Network, the Palliative Care Health Services Delivery Framework in the Community (“The Delivery Framework”), and the Ontario Palliative Care Competency Framework.
