SUMMARY:

The Patient Advocate (I) will perform duties and responsibilities as outlined below, as well as tasks assigned by the Care Manager. These tasks include but are not limited to performing recalls according to the recall procedure, running reports independently, reviewing data and contacting patients while ensuring the patient has timely access to care.

 

The Patient Advocate (I) works within the context of a primary care medical home, from a team approach with the philosophy and guidelines of the Patient Centered Medical Home and Trauma Informed Care and in continuous partnership with families and the health care team to promote timely access to needed care, comprehensive and continuity of care, and the enhancement of patient well- being.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Coordinate care within SCHC for patients before, after, and between visits as ordered by providers.
  2. Educate patients on care teams, resources, and processes at SCHC that impact their care.
  3. Generate and review reports to ensure patients are getting care as ordered by providers and to identify and address gaps in care.
  4. Generate biweekly reports of actions due based on ticklers and overdue incomplete orders.
  5. Review reports and orders for accuracy prior to contacting patients. Contact patients proactively to schedule office visits, nurse visits, and lab draws (internal and external orders).
  6. Retrieve and upload information from other medical entities as appropriate.
  7. Use appropriate software to track care that facilitates data collection and improvement processes.
  8. Use appropriate documentation to contribute to care management documentation.
  9. Participates in QI initiatives.
  10. Attends clinical meetings.
  11. Assists to establish a respectful and compassionate environment for patients, families, and friends, using the Patient Centered medical Home and trauma informed care guidelines.
  12. Promote safe and clean working environment by following procedures, rules and regulations.
  13. Maintains positive working relationship with all staff in all departments.
  14. Supports collaborative relationships with the patient’s medical and care team.
  15. Assist Patient Advocate II as assigned.
  16. Other duties as assigned.
 EDUCATION and/or EXPERIENCE:

 

Graduation from high school or the equivalent. Two years of appropriate office work experience, including at least six months performing duties similar to most of those specified above.
