BENEFITS SUMMARY
INSURANCE PROGRAMS
HEALTH INSURANCE
Employees are eligible at the first of the month following 30 days of employment. The plan contains two options, and the Company pays a portion of the premium.
DENTAL INSURANCE
Employees are eligible at the first of the month following 30 days of employment. Discount and traditional options are available.
VISION INSURANCE
Employees are eligible at the first of the month following 30 days of employment. The plan includes allowances for frames and contact lenses for all participants.
LONG TERM DISABILITY INCOME PLAN
Employees are eligible following one year of full-time continuous employment.
FLEXIBLE SPENDING PLAN
Health care expenses can be deducted before taxes and paid for via a Flexible Spending Account.
WORKERS' COMPENSATION
The Company provides workers' compensation insurance for a work-related injury or illness.
EMPLOYEE ASSISTANCE PROGRAMS
Employees, regardless of health insurance elections, have access to a limited number of counseling sessions provided at no charge.
GROUP LIFE INSURANCE
Employees are eligible at the first of the month following 90 days of employment. Premiums are paid for by the company for employee coverage only. Coverage amount depends on age, tenure, and salary.
ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE
Employees are eligible at the first of the month following 90 days of employment. Premiums are paid for by the company for employee coverage only. Employees may elect coverage options for spouse and dependents.
RETIREMENT PLANS
401(K) PLAN
Employees are eligible to participate in January or July after 30 days employment. 401(k) and Roth 401(k) options are available, and employees 100% vested immediately. The plan offers a 4% company match on a dollar-for-dollar basis.
DEFINED CONTRIBUTION PLAN
Employees are eligible to participate in January or July after 30 days employment. The Company contributes on an annual basis, and employees are vested after 3 years of service.
OTHER BENEFITS
PAID TIME OFF (PTO)
Accruals begin immediately upon hire. New fulltime employees earn up to 25 days per year.
HOLIDAYS
Approximately 9 paid holidays each year.
